Rapid discharges
Advice for ward staff to follow - This is only a
guide and may not be inclusive
- Confirm that PPC is home. Discuss with patient and family.
- Contact District Nurses/GP and Community Macmillan Nurses. Ask
re: care provision/marie curie/equipment.
- TTO's (pathway drugs) & authorisation letters.
- Obtain pre-paid padded envelope for syringe driver and
notification of loan form from equipment co-ordinator (if
required).
- Order home oxygen (if required)
- Order transport, photocopy DNR.
- Supply immediate supplies ie: catether bags/stand,
syringes/needles, pads, gloves etc.
- Refer to hospice (deferred) if required/appropriate.
- Fax written D/N referral and CMN referral with copies of
authorisation letters.
