Home oxygen therapy service
As a result of NICE guidelines, a single,
dedicated oxygen supply company has been appointed to provide a
fully integrated service to each of the regions in England and
Wales. This is aimed at providing patient's with more appropriate
treatment by improving assessments and monitoring results.
Prescriptions or FP10's have now been replaced by a Home Oxygen
Order Form (HOOF).
HOOFS will state the need (short burst,
long-term, ambulatory or lightweight), the flow rate and the hours
of use each day. They will not specify the equipment to be
supplied.
Home oxygen will need to be arranged in this way to facilitate
hospital discharge, however we will request a patients GP takes
over the monitoring/assessment process.
Any registered healthcare professional can place an order.
Guide to ordering for ward/medical teams:
- HOOF and HOCF (consent) should be completed with
the patient present.
- These forms MUST be completed in full - any omissions will
result in delay or non supply.
- Fax the HOOF to the relevant supply company, for
the majority of patients this will be Air Products.
- Contact suppliers to confirm order received and adequately
completed.
- Return original HOOF
and HOCF to the patients medical notes.
A PHOTOCOPY OF THE HOOF MUST THEN BE FORWARDED TO THE
COMMUNITY LINK TEAM. It will then be forwarded to the
patients GP, PCT and the Trust Clinical Oxygen Lead.
PCT's will meet the costs of this service, orders for emergency
delivery and/or next day/hospital discharge will be charged at a
higher rate.
