Discharge check list

This checklist is to enable you to identify which patients have complex needs.

  • Does the patient live alone? Do they live in suitable accommodation?
  • How mobile is he/she? Refer to Physiotherapist if concerned.
  • Do they have stairs? Are they likely to require downstairs living?
  • Have they got access to toilet facilities? Is a commode/urinal required? Who will empty this?
  • If they require District Nurses/carers how will they gain access? Can the person open the door?
  • Is a pressure relieving mattress/cushion required? Provide up-to-date waterlow score.
  • Have they got any caring responsibilities? Refer to social services
  • What support do they have? Do they already have carers?
  • Do they need assistance with personal cares/meal provision? Refer to social services.
