Role of the team
- To assess patients 'Nursing' needs
- Ordering of appropriate 'Nursing' equipment
- Processing of Home Oxygen orders
- Referral to / Liaison with Community District Nurses, Macmillan
Nurses, Hospices and Respite Teams (only patients we are involved
with).
- Assessment of eligibility against NHS Continuing Healthcare
Criteria and completion of checklist.
- Co-ordination of NHS Continuing Healthcare process and liaison
with PCT's
- Resource for Community Teams, Ward Staff and Medical
Teams.
- Emotional Support to Pateint /Relatives.
- Identification of Appropriate Discharge Destination
- Rapid Discharges

Nursing Equipment
The team assess for appropriate equipment and either order
direct or via District Nurses. Please be aware delivery is likely
to take 2-5 days. We order nursing equipment such as:
- Hospital Beds (need to meet criteria)
- Pressure Relieving Mattresses/Cushions
- Commodes
- Nebuliser Machines
- Oxygen (processing of)
- Continence Products
