Study Sponsor Feedback Form

As part of our commitment to improving the service offered to sponsors conducting clinical trials at The Christie, we are asking for feedback on various aspects of the clinical trial process.

The results of the feedback will be used to assess and improve the services we offer and how well we meet the needs of you, as sponsors of research.

The forms are divided into 3 sections:

• Set-up feedback

This should be completed once R&D approval has been granted.

• Active feedback

This can be completed at any time during active recruitment to the trial or at last patient recruitment.

• Closure feedback

This should be completed after the end of study report has been submitted.
